Architectural request

Is this the official form?

It reproduces the Association's own architectural request form field for field, in the same order, addressed to the same office — so what you submit reads exactly like their form, just filled in here instead of by hand.

Legally, that match is not even required. Civil Code § 4765 does not require a homeowner to use a particular piece of paper — it requires the Association's procedure to be fair, reasonable and expeditious, with the deadlines described above. If the Board has since adopted an operating rule specifying a required form, ask for a copy of that rule; under Civil Code § 4360 a rule change requires 28 days' notice to members before it can take effect.

If you would still rather use their exact document, request a blank copy from management — Civil Code § 4765(c) requires the Association to give annual notice describing its architectural process and include a copy of the application.
